Physical Therapist - Full Time - LIFE - St. Joseph of the Pines

LIFE St. Joseph of the Pines, located in Fayetteville, North Carolina is a Program of All-inclusive Care for the Elderly (PACE). PACE is a unique program which can assist elders to live safely at home or with their loved ones with the help of a team of health care professionals. PACE provides a full spectrum of health care services for frail elderly individuals certified to require nursing home care, and is centered around the belief that it is better for seniors with chronic care needs and their families to be served in the community wherever possible.

St. Joseph of the Pines is currently seeking a Physical Therapist for our LIFE facility in Fayetteville, North Carolina. The primary purpose of the Physical Therapist is to provide clinical physical therapy evaluation and treatment for program participants, on acute, restorative or maintenance levels as needed by Day Center, Nursing Home or in-home setting.

Duties include, but are not limited to:

  • Coordinates with the Interdisciplinary Team to develop a comprehensive plan of care for each participant.
  • Educates staff members in body mechanics, use of equipment, transfers, gait patterns, and methods to maximize safety and functional abilities in performing ADL’s.
  • Maintains the Therapy Department and its equipment as required by plant operations, manufactures recommendations, and state and federal agencies.
  • Assists in recommending physical therapy policies and procedures concerning services provided.
  • Performs in person comprehensive physical therapy for initial acceptance into the LIFE Program, as well as annual including problem identification and care planning.
  • Evaluates participant’s needs. Follows-up with participants during treatment, explaining precautions to take. Discusses condition and treatment plans and recommends solutions to participants care problems with Interdisciplinary Team.
  • Evaluates, charts and reports on participant’s progress, documents physical therapy notes in the participant’s medical record according to LIFE St. Joseph of the Pines charting requirements.
  • Educates and instructs participants and family members regarding in-home exercise programs: gait training; transfers; Activity of Daily Living (ADL); home modifications; care and use of equipment.
  • Conducts assessment and coordination of home modification program including installation of equipment and other changes to support independence and safety.

Education and Experience:

  • Must be a graduate of a physical therapy curriculum.
  • Must have current State of North Carolina Physical Therapy license.
  • Minimum of one-year experience working with a frail or elderly population required, no exceptions.
