Lead Physical Therapy Assessment Specialist position summary

Our Lead Physical Therapy Assessment Specialist provides professional therapy care to the organization’s clients as prescribed by the physician, and/or requested by the client/family, and in compliance with the state’s Therapy Practice Act, any applicable licensure/certification requirements, and the organization’s policies and procedures.  The Lead Therapist is proficient in OASIS assessments/documentation, and is fully responsible for the client assessment, care planning development, obtaining orders, initiating telemonitoring, and communicating to the care team. The Lead Therapist is able to meet all productivity standards, and is knowledgeable regarding evidence-based care and payer requirements. Provides functional support to PTAs and Home Healthcare Aides as needed

Minimum qualifications

  • Bachelor’s degree with progressive experience or Master’s degree
  • Licensed to practice as a physical therapist in Iowa
  • Home healthcare experience required, with demonstrated competence/proficiency with OASIS
  • Must have current driver’s license and reliable transportation to and from work site.
