Trinity Health LIFE New Jersey (formerly LIFE at Lourdes),  has a management career opportunity for an Home Care Manager, RN for its location in Pennsauken, New Jersey. Programs of All-Inclusive Care for the Elderly (PACE) coordinate and provide all needed preventive, primary, acute and long term care services so that older individuals can continue living in the community. PACE is an innovative model that enables individuals who are 55 years old or older and certified by their state to need nursing home care to live as independently as possible. Through PACE, today’s fragmented health care financing and delivery system comes together to serve the unique needs of each individual in a way that makes sense to the frail elderly, their informal caregivers, health care providers and policy makers.  Many PACE participants get most of their care from staff employed by the PACE organization in the PACE center. PACE centers meet State and Federal safety requirements and include adult day programs, medical clinics, activities, and occupational and physical therapy facilities.

The Home Care Nurse Manager, RN is responsible for coordinating and supervising nursing services provided to the participants in the PACE Organization. Services may be delivered in the day center or in participants home.   Supervision includes overseeing and providing leadership to a team of registered nurses, licensed practical nurses and home health aides.

General responsibilities will include:

  • Responsible for the effective operation of the center as the focal point of the PACE model, including the routine requirements to meet participant needs; involves rigorous communication and coordination with all departments.
  • Participate as a member of the interdisciplinary team (IDT), including completion of initial medical history, physical exam, and functional nursing assessments of each new participant and semi-annual, annual, and unscheduled assessments; communicate changes in participant health or functional status to the interdisciplinary team members, and participate in development of the plan of care and coordination for 24-hourcare delivery.
  • Actively serves on the Quality Assessment and Process Improvement (QAPI) Committee.  Involved in the development and implementation of QAPI activities.
  • Responsible for supervision of center and/or in-home staff, including orientation, competency assessment, training, development, and performance appraisal.
  • Coordinate participant care with outside contracted service providers, including hospitals, nursing facilities, assisted living facilities, lab, oxygen, etc. 
  • Provide on-call coverage during center hours of non-operation.

Minimum Qualifications:

  • Must possess a current, unencumbered active license to practice as a Registered Nurse in the state of New Jersey.  Active CPR certification required.
  • Bachelor's of Science in Nursing, strongly preferred.
  • Previous supervisory experience to include performance management of personnel, strongly preferred
  • Minimum of three (3) years' health care experience with emphasis in geriatrics.
  • Must have the ability to perform standard clinical procedures according to established protocols and possess initiative, solid problem solving skills and a high level of accuracy and thoroughness in documenting the delivery of services.
  • Must possess strong interpersonal skills in order to interact in extremely delicate, sensitive and/or complex situations.
  • Candidates should have excellent communication skills, with the ability to communicate with multiple levels in the organization.
  • Must possess strong analytical and organizational skills. Must have the ability to meet strict deadlines with a high level of accuracy, ability to prioritize tasks in a highly automated setting and possess strong interpersonal skills.
