Healthcare Coordinator - Full-Time- LIFE -St. Joseph's of the Pines

LIFE St. Joseph of the Pines, located in Fayetteville, North Carolina is a Program of All-inclusive Care for the Elderly (PACE). PACE is a unique program, which can assist elders to live safely at home or with their loved ones with the help of a team of health care professionals. PACE provides a full spectrum of health care services for frail elderly individuals certified to require nursing home care, and it is centered around the belief that it is better for seniors with chronic care needs and their families to be served in the community wherever possible.

LIFE St. Joseph of the Pines is currently seeking a Healthcare Coordinator. Under the supervision of the Assistant Director, the Healthcare Coordinator is responsible for completing health assessments for initial acceptance into the LIFE Program, to include problem identification and care planning. In addition, the Healthcare Coordinator oversees and manages the daily operations of the Day Center. This includes ensuring that appropriate personnel are trained and perform their functions within the organization, as well as ensures operations are in compliance with all governing regulations. 

Duties and responsibilities include, but are not limited to:

  • Working knowledge of the PACE regulations
  • Broad knowledge base of health care problems of the frail and elderly
  • Skilled in identifying problems, recommending solutions, utilizing creative problem-solving methods for complex issues
  • Directs and supervises the Day Center clinical staff
  • Ensures that a safe and clean environment is maintained within the Day Center setting for staff and program participants
  • Maintains professional affiliations and any required certifications
  • Provides initial and ongoing training for all nurses and CNAs
  • Educations staff members to emergency procedures, Infection Control prevention, and provides information about health concerns and conditions of participants
  • Engages in team building, and coaching and mentoring staff
  • Performs in-person health assessment for initial acceptance into the LIFE Program, including problem identification and care planning as needed.
  • Continuously evaluating the participants' changing needs and making necessary program adjustments
  • Monitors participants' response to medical treatment plan and nursing interventions and revises plan of care as necessary

Education and experience:

  • Current North Carolina Registered Nurse (RN) License
  • CPR certification is required.
  • Graduate of a school of professional nursing. Bachelor of Science in Nursing preferred.
  • Minimum of 5 years of health care experience, and 2 years of management experience, preferably in a geriatric care setting
  • 1 year minimum experience working with the frail elderly population is required. No exceptions.
