Community Treatment Team Nurse/Service Coordinator

The Community Treatment Team Nurse (CTT) will provide medication management, case management and psycho-education services to clients with mental health and substance use diagnoses. Assists adults in accessing physical health services, building support networks, and providing health education and medication management. Participates as a member of the Community Treatment Team (CTT) which includes collaboration with the team psychiatrist and multidisciplinary team members as well as other healthcare providers. Maintains clinical records according to organization policy and procedures.

Minimum Qualifications & Requirements:

Registered Nurse license and 2 years of Community-Related nursing experience OR Bachelors of Science in Nursing and 1 year of Community-Related.

Valid driver's license, access to a reliable vehicle, and computer skills are also required.

Work Schedule:

Primarily, Monday-Friday, 8:00 am - 4:00 pm, with rotating evening and on-call as part of a team.  Some weekend and holidays may be required.
